Why it works

The rotator cuff's job is to keep the ball of your upper arm centered in its socket while bigger muscles like the chest and lats handle the heavy pulling and pushing. Cable external rotation loads that job directly: with your elbow pinned to your side, the cable pulls your forearm back toward your body, and the infraspinatus and teres minor have to work against that pull to rotate your arm outward. Because the cable keeps tension constant through the whole arc, unlike a dumbbell that goes slack near the top, the cuff stays under load for the entire rep instead of just the hardest point.

How to do it

  1. Keep your elbow glued to your ribs for the entire rep.
  2. Rotate your forearm outward and away from your body, keeping the 90-degree bend at the elbow.
  3. Stop when your forearm is roughly pointing forward, away from the machine.
  4. Pause for a beat at the end of the rotation.
  5. Return the handle slowly to the starting position, resisting the cable rather than letting it snap back.
  6. Complete all reps on one side before switching arms.

Form cues

Breathing: Exhale as you rotate your forearm outward, inhale as you control it back to start.

Common mistakes

The elbow drifts away from the ribs as the weight gets heavier.

Fix: Drop the weight until you can keep the elbow pinned for every rep; this exercise only works if the shoulder blade and torso stay still.

Twisting the torso to help finish the rotation.

Fix: Face the machine squarely and let the small rotation come from the forearm alone, not a body turn.

Letting the cable snap the arm back to the start.

Fix: Control the return at the same speed as the outward rotation; the eccentric is where most of the stabilizing work happens.

Rotating too far past a comfortable range.

Fix: Stop when your forearm points roughly straight ahead; forcing extra range past that point stresses the front of the shoulder for no real benefit.
